The signed Enrollment Summary and Service Schedule controls the selected tier, term, monthly rate, exact Hollis-provided services, Hollis-billed add-ons, and any separately billed third-party items. The memberships page shows the standard tier comparison. Independent medical providers, labs, imaging, prescriptions, and other third-party clinical items are billed separately by those providers and are not Hollis Health membership services.

Your first visit is designed to clarify your goals, health history, lifestyle, and the services that may fit your needs. Depending on your enrollment path, this may include baseline measurements, referral-network coordination, and a Hollis Health onboarding session. Exact timing and included services are confirmed before enrollment.

Hollis Health operates as a membership-based program and does not bill insurance directly. We may provide receipts or documentation on request, but reimbursement, HSA/FSA eligibility, and out-of-network benefits are determined by your plan administrator, insurer, or tax advisor.
